Digital care with medical forms.

Use the function to create questionnaires for specific appointment reasons. This makes the assessment of status easier and helps care providers to establish an overall picture of the patient’s care requirements. The form can be set up with ready response options to selected questions or by you choosing that certain questions should be answered in free text.
We will help you to create your own forms when you sign up, by digitising the forms that you already use.

Digitising other types of forms

Some of our customers use the questionnaire to digitise the business’s health surveys. By making the questionnaires available in BDC Care, they can ask the patient to respond to them regardless of whether they are meeting in person or by video.

Third party cookies

Third party cookies are set by a party other than the one that is responsible for the website, in this case therefore by a company other than CAB. The purpose of these cookies is to track how visitors move between different websites. These cookies gather information about your activities and interests on our website, and are used by our partners to show you tailored advertisements about our products when you are visiting other websites.

In order to understand and analyse how visitors use our website, we use the tool Google Analytics. The information generated by Google Analytics cookies is stored on servers at Google, and your IP address is truncated. We use these cookies to better understand how the website is being used so that we can improve it. The information is also used for marketing purposes. Google can transfer the saved information to a third party if this is required by law or in those cases where a third party processes the information on Google’s behalf.

We also use the Facebook pixel in order to measure advertisement conversions and use remarketing for advertisements on Facebook.
